Job Summary:In accordance with agency policy and state/federal regulations, provides independent, age-appropriate physical therapy to patients in their place of residence. Teaches patient/family and caregivers preventative and rehabilitative procedures. Develops and coordinates the plan of care and provides case management.Essential Responsibilities:Evaluates patients and interprets evaluation findings to determine the nature and extent of dysfunctionDetects early warning signs of complications including medical assessments of skin integrity and mentationAbility to differentiate emergent versus non emergent patient care status and escalates appropriatelyProviding initial and ongoing assessment to determine level of functioningEvaluate the patient comprehensively, not just body part or functional ability.Establishes treatment goals and plans treatment to achieve established goalsInitiates discharge planning for physical therapyImplements the of care through direct treatment of the patient other health care personnel as appropriate.Provides advice and education to patient and familyRegularly reassesses clinical symptoms to determine effectiveness of treatment, progress of patients towards goals, and the need for modifying goals and treatment.Discharges patient from physical therapy when the patient has met their treatment goals.Provides accurate, timely documentation of physical therapy patient managementParticipates in appropriate and approved educational and clinical research activities in physical therapyCompletes OASIS documentation according to guidelines.Coordinating with pharmacy, inpatient physical therapy, outpatient departments.Assists in the development, implementation, and review of departmental patient care programsPerforms other duties as needed in the areas of staff education, peer reviews, departmental operations, and departmental projectsSupervises the work of Home Health Aides and Physical Therapy Assistant according to regulation and agency policy.Demonstrates on-going progress toward meeting the criteria for professional practiceConsistently supports compliance and the Principles of Responsibility (Kaiser Permanentes Code of Conduct) by maintaining the privacy and confidentiality of information, protecting the assets of the organization, acting with ethics and integrity, reporting non-compliance, and adhering to applicable federal, state and local laws and regulations, accreditation and licenser requirements (if applicable), and Kaiser Permanentes policies and procedures.Basic Qualifications:ExperienceMinimum of two (2) years experience in physical therapy.EducationGraduate of an accredited physical therapy curriculum with a Bachelors, Masters or Doctoral degree with certification in Physical Therapy or foreign-trained equivalent.License, Certification, RegistrationDriver's License (California)Physical Therapist License (California)Basic Life SupportNational Provider IdentifierAdditional Requirements:Clinically skilled to detect early warning signs of complications including medical assessments of skin integrity, mentation.Ability to differentiate emergent versus non emergent patient care status.Ability to look at patient holistically, not just body part or functional ability.Competency to remove staples/sutures in the home.Commitment to Service Orientation.Effective written and oral communication skills (in English).Good interpersonal skills.Must be able to work in a Labor/Management Partnership environment.Clear driving record for the past two (2) years and proof of automobile insurance as required by law.Preferred Qualifications:Experience in an automated clinical information system preferred.1-year experience in home health agency preferredOASIS CertificationNotes:Weekends on rotation.J-18808-Ljbffr
