Physical Therapist (PT) Responsibilities: Provide physical therapy services to residents in our skilled nursing and rehabilitation center, including initial evaluations, treatment plans, and progressive rehabilitation goals Conduct daily rounds to assess the physical needs of residents and develop individualized treatment plans to address their unique needs Collaborate with other healthcare professionals, including nurses, occupational therapists, and speech therapists, to ensure comprehensive care for our residents Develop and implement effective treatment plans that promote optimal physical function, strength, and mobility Participate in multidisciplinary team meetings to discuss resident progress and develop effective care plans Maintain accurate and detailed records of resident treatments and progress Per Diem Opportunity: As a Per Diem/PRN Physical Therapist (PT) , you will be expected to work on an as-needed basis, with flexible scheduling to meet the needs of our community.
