Part-Time/PRN Physical Therapist (PT)
We are seeking a Part-Time/PRN Physical Therapist (PT) to join our home health team in Chicago, IL Area. One Home Health: Overview One Home Health is a full-service, Medicare-certified home health agency licensed in Illinois and Indiana. Our mission is to deliver compassionate, personalized in-home care through a highly trained, world-class team committed to exceptional clinical outcomes and patient satisfaction.

What You'll Do
- Perform in-home physical therapy evaluations and treatments
- Develop and implement individualized rehabilitation plans of care
- Monitor patient progress and adjust treatment plans as needed
- Educate patients and caregivers on exercises, mobility, and safety
- Document visits accurately and efficiently in accordance with agency guidelines
- Communicate patient progress with the interdisciplinary care team
- Ensure compliance with clinical, documentation, and regulatory standards
What We're Looking For
- Licensed Physical Therapist (PT)
- Doctorate in Physical Therapy (DPT)
- Home health experience preferred, but not required
- Strong communication and documentation skills
- Ability to work independently in a home-based setting
- Professional, dependable, and patient-focused approach
- Physical ability to perform in-home visits
