245D Designated Coordinator / Designated Manager (DC/DM)
Good Hands Home Health Care LLC is seeking a qualified and experienced Designated Coordinator/Designated Manager (DC/DM) to oversee our 245D services program and ensure compliance with Minnesota DHS licensing requirements.
Responsibilities
- Oversee daily operations of 245D services.
- Ensure compliance with Minnesota Statutes Chapter 245D.
- Supervise, train, and support Direct Support Professionals (DSPs).
- Coordinate services with case managers, guardians, families, and support teams.
- Review and maintain client documentation, service plans, and progress notes.
- Conduct quality assurance reviews and monitor program compliance.
- Manage incident reporting and follow-up activities.
- Participate in admissions, assessments, and service planning meetings.
- Assist with licensing reviews, audits, and corrective action plans.
Qualifications
- Must meet Minnesota 245D qualifications for Designated Coordinator and/or Designated Manager.
- Bachelor's degree in Human Services, Social Work, Psychology, Healthcare Administration, or related field preferred.
- Experience working with individuals with disabilities, mental health needs, or waiver services.
- Supervisory experience preferred.
- Strong knowledge of Minnesota DHS regulations and licensing requirements.
- Excellent organizational, communication, and leadership skills.
