JourneyCare Hospice is seeking a Hospice Social Worker for our growing team! Provide an assessment in the patient’s residence and assist when this is not safe, requiring another plan. Demonstrate exceptional service to patients, families, referral sources, co-workers, and other customers. Assist team members as needed to maintain this standard. Counsel patients and families/caregivers and staff as needed regarding stress and other coping difficulties. Maintain accurate and timely documentation of patient services to reflect the plan of care. Assist with applications for Supplemental Security Income, Hospice, Medicaid, and Medicaid Waiver. Obtain resources from community social, health, pharmaceutical and welfare agencies to meet patient/family needs. Serve as a liaison between patients and families/caregivers and community agencies.
