Hospice RN Case Manager

Four RN Case Managers are needed for a 13-week hospice assignment with an ASAP start. This role requires extensive driving between Santa Cruz, Capitola, Salinas, and Monterey, with mileage paid while on the clock. Candidates must be comfortable managing a hospice caseload, providing direct patient care, and supporting families through end-of-life needs.

Hours per Week: 40

Contract Length: 13 weeks

Charting System: Matrix (on provided iPad)

Interview: Virtual or phone

Caseload: Hospice field-based

Responsibilities

Respond to clinical referral information and address patient/family inquiries

Assess hospice eligibility and collaborate with Clinical Director

Provide direct hospice nursing care focused on comfort and quality of life

Deliver evidence-based nursing interventions

Educate patients and families on care plans and symptom management

Participate in shared call rotation

Drive between multiple service areas daily

Required Experience & Certifications

Minimum 6 months hospice or home health RN experience

Active RN license

CPR certification

Must be computer-proficient and comfortable with EMR

Valid driver's license, insurance, and reliable transportation

Bachelor's degree in nursing OR 1 year RN experience within the last 3 years

Compensation

Local RN Hospice Case Manager

Weekly Pay Package: $2,400/week Breakdown:

Hourly Pay: $60/hr

Total Weekly Package: $2,400/week
