RN - Case Manager

Case Management Registered Nurse (RN)

We are seeking a qualified Case Management Registered Nurse (RN) to join our team. This is a 13-week contract position with a start date as soon as possible.

Position Details

• Shift: 5x8 Days (07:30 - 16:00)
• Guaranteed Hours: 72 hours per 2-week period
• Floating: Requires floating between various units
• Weekends: Required every other weekend

Responsibilities

• Provide effective case management, including discharge planning and care coordination.
• Collaborate with multidisciplinary teams to ensure comprehensive patient care.
• Work with patients across various departments including Med-Surg, LLOS, and ED.
• Assist with the transition of care and ensure follow-up resources are in place.

Qualifications

• 1 year of acute care experience is required.
• Experience in discharge planning and care coordination is essential.
• Diverse case management experience, particularly in Med-Surg, LLOS, and ED, is highly preferred.
• Must hold a valid Washington State RN License.
• Current BLS certification is required.
• First-time travelers are encouraged to apply.

Additional Information

• This role will require working in all areas of the hospital, including multiple locations within the region.
• Parking costs will be covered by the facility.
• Charting will be conducted using EPIC.
