- Conduct comprehensive patient evaluations in either inpatient or outpatient environments and formulate tailored treatment plans
- Implement physical therapy interventions focused on enhancing mobility, building strength, and improving overall function
- Work collaboratively with nurses, physicians, case managers, and fellow rehabilitation specialists to maximize recovery outcomes
- Guide patients and families through therapeutic goals, discharge requirements, and maintaining home exercise routines
- Record evaluations, therapies, and progress diligently adhering to facility standards and compliance protocols
- Engage in care team discussions and interdisciplinary meetings to ensure cohesive patient care
- Doctorate in Physical Therapy (DPT) or a related degree from a recognized institution
- Active state licensure as a Physical Therapist
- CPR certification preferred
- Additional specialized certifications (such as in neurology or geriatrics) are advantageous but not mandatory
- Relevant clinical experience highly preferred
- Ability to assess and treat patients with intricate medical histories and mobility challenges
- Excellent communication and collaboration skills
- Comfortable utilizing digital charting and clinical documentation software
