Case Management RN - Ortho Care Coordinator

This is a 13-week contract position.

Monday through Friday, 8:00 AM - 4:30 PM or 8:30 AM - 5:00 PM; no weekends.

The Case Management RN, specializing as an Ortho Care Coordinator, will be responsible for facilitating the discharge planning process, creating navigation plans, and providing education to patients following orthopedic surgery. The ideal candidate will have experience in ortho care coordination with familiarity in utilizing Parachute and Careport software.

Responsibilities:

• Develop and implement discharge plans for patients.
• Educate patients and their families regarding post-surgery care.
• Coordinate navigation plans to optimize patient recovery.
• Work alongside a multidisciplinary team to ensure appropriate patient transitions.

Required Experience / Certifications / Licensure:

• Minimum of 3-5 years of relevant clinical experience in medical-surgical, ICU, or ER settings preferred.
• Experience with case management, discharge planning, and navigation preferred.
• Current New York State RN license required.
• BLS certification (AHA or ARC) required; ACLS preferred.
• HC-PRI Certification and PRI screen preferred; CCM certification is a plus.
