Mobile Physical Therapist - Home-Based Post-Discharge Care

Northwestern Memorial Hospital

Northwestern Medicine is seeking a Physical Therapist for our Mobile Physical Therapy Team in Illinois. This role delivers skilled outpatient therapy directly in patients’ homes, with telehealth supported as appropriate, focusing on safe transitions after discharge from inpatient rehab.
