Key Responsibilities:

• Conduct comprehensive assessments to determine the physical therapy needs of each patient.
• Develop personalized treatment plans aimed at improving mobility, strength, and overall physical function.
• Implement therapeutic exercises and techniques to address patient-specific goals.
• Educate patients and their families on home exercise programs and techniques to ensure continuity of care.
• Collaborate with other healthcare professionals to provide holistic and coordinated care.
• Document patient progress and modify treatment plans as necessary to achieve optimal outcomes.

Qualifications

• Valid state licensure as a Physical Therapist.
• Proven experience in home health or a related healthcare setting is preferred.
• Strong interpersonal and communication skills.
• Ability to work independently and manage a flexible schedule.
