Responsibilities include monitoring patient conditions and providing necessary interventions. The RN collaborates with a multidisciplinary team to ensure optimal patient outcomes. Effective communication and patient advocacy are essential in this role. Additionally, the RN will educate patients and families on post-discharge care and resources.
