Serves as a resource to other healthcare professionals including pharmacy students and residents and ensures that patient care meets standards of safety and effectiveness. Oversees the duties of the pharmacy technicians and interns./n /n /n /n Job Responsibilities and Requirements: /n PRIMARY RESPONSIBILITIES /n /n - Ensures safety and appropriateness of medication therapy through order verification, dispensing, home medication reconciliation, chart review, patient counseling, lab value monitoring and dose/therapy adjustment as applicable. Documents medication related plans and interventions in electronic medical record. /n - Provides clinical consultation delivering best-practice, evidence-based and complete information to practitioners. Makes recommendations and encourages safe and cost-effective pharmacotherapy for patients through multi-disciplinary collaboration. /n - Oversees the medication dispensing process to ensure accurate and timely delivery of optimal pharmacotherapy to patients. This includes supervision of pharmacy technicians and/or interns who support the role of the pharmacist in the drug delivery system. /n - Precept pharmacy students and residents, as applicable. /n - Works in a constant state of alertness and safe manner. /n - Performs other duties as assigned. /n /n /n EDUCATION /n /n - Pharmacy degree (Pharm.D; Bachelor of Pharmacy) from accredited school of pharmacy /n /n /n EXPERIENCE /n /n - No experience required /n /n /n PHYSICAL REQUIREMENTS /n /n - Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs. /n - Frequent sitting, standing, walking, reaching and repetitive foot/leg and hand/arm movements. /n - Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors. /n - Frequent use of hearing and speech to share information through oral communication. Ability to hear alarms, malfunctioning machinery, etc. /n - Frequent keyboard use/data entry. /n - Occasional bending, stooping, kneeling, squatting, twisting and gripping. /n - Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs. /n - Rare climbing. /n /n /n REQUIRED PROFESSIONAL LICENSE AND/OR CERTIFICATIONS /n /n Resuscitation Certification Requirement: Basic Life Support Healthcare Provider (BLS HCP) required within 90 day of employment.
