Mileage will be reimbursed @ $0.55/mile. RTO Restrictions: No more than 7 days Guaranteed Hours: 3 shifts per 13 weeks. The cancelled shifts must be the regular shift length to which the Contractor Personnel is assigned Special Requirements: Monday-Friday, 8:00am-4:30pm, this RN Case Manager will support a patient load of 15-18 patients. Must have Hospice RNCM experience. HOSPICE: Home Care / Nursing Home: visit nurses, med management, supply management, DME management, wound care, teaching Admissions, triage visits, routine visits. Provide care and support to patients and families at end of life.
