Sentara Health Plans

Sentara Health Plans is seeking an Integrated Nurse Case Manager for the South West Virginia Region. This full-time role involves case management services, clinical assessments, and coordination of care. Ideal candidates will have an RN license, at least 3 years of nursing experience, and familiarity with chronic illness management.
