Wound Care Coordinator Requirements: Active Nursing License (LPN/RN) as required by the state Current CPR certification Minimum two (2) years of experience as an RN or LPN Ability to lift and move patients and items up to 75 lbs Strong vision abilities including close, distance, peripheral, and depth perception Excellent verbal and written communication skills in English Ability to cope with the mental and emotional stress of the position Strong attention to detail and ability to prioritize wound care needs As a Skilled, Compassionate Wound Care Coordinator, You Will: Utilize the nursing process for assessment, planning, implementation, and evaluation of wound and skin care Conduct regular wound care rounds, ensuring timely documentation and tracking of resident skin conditions Assess resident skin integrity and document medical history related to wound care Implement preventative wound care strategies in resident care plans Prioritize and execute nursing interventions for wound management Communicate with physicians and interdisciplinary teams regarding wound treatment needs Initiate nutritional interventions to support wound healing Perform direct resident care in compliance with facility policies and procedures Ensure residents with incontinence receive appropriate pericare and preventative treatments Maintain a safe and clean work environment, adhering to infection control protocols A Workplace That Cares About You!
