" Job Description & Requirements Jackson Therapy Partners is seeking a travel Skilled Nursing Facility (SNF) Physical Therapist for a assignment in Souderton, Pennsylvania. The role involves helping patients recover from injuries, manage chronic conditions, and treat geriatric populations, including residents in memory care and personal care settings. Specialty Physical Therapist Discipline Therapy Start Date 05/18/2026 Duration 13 weeks Hours 40 hours per week (8 hour shifts) Shift 8 hours Employment Type Travel Responsibilities Provide physical therapy services to SNF residents, addressing acute injuries and chronic conditions. Develop and implement individualized treatment plans for geriatric patients. Collaborate with interdisciplinary teams to optimize patient outcomes. Document patient progress using NetHealth EMR (preferred). Supervise or collaborate with Physical Therapy Assistants as needed. Qualifications Minimum: Bachelor's, Master’s, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA. Active Pennsylvania state license required to start the assignment. BLS certification from AHA or ARC preferred. Experience working in Skilled Nursing Facility (SNF) settings required. Strong clinical skills and prior experience treating geriatric populations, including memory care and personal care settings. Familiarity with NetHealth EMR preferred. Experience supervising or collaborating with PTAs preferred. Ability to provide a minimum of 35 hours per week guaranteed for the assignment.
