ICU Registered Nurse

Department: MS/ICU/IMC RN Certifications Required: ACLS, BLS, PALS Number of Beds: 20 Nurse to Patient Ratios: - MS: 1:5 during the day, 1:6 at night - IMC: 1:4 - ICU: 1:2 - Pediatric: 1:3-4 Common diagnoses/types of patients include: - Elderly population with cardiac issues such as CHF exacerbation, Afib RVR, bradycardia, syncope. - Pulmonary cases including pneumonia, COPD exacerbation, and respiratory failure post-op. - Neurological cases involving falls, TIA, stroke, and detox. - Skin conditions such as cellulitis and decubitus ulcers in paraplegic patients. - Gastrointestinal issues including GI bleed, SBO, lysis of adhesion, pancreatitis, cholecystitis, intestinal resections, and diarrhea. - Genitourinary conditions such as UTI, hematuria, TURP, hydronephrosis, kidney stones, AKI, ileal conduit, and nephrostomy tubes. - Orthopedic cases including hip fractures and surgeries of the shoulder, knee, or hip. - Sepsis. - Outpatient needs such as transfusions or long-term antibiotic infusions. - Pediatric stable patients with respiratory illness for IV medications, post-operative appendicitis, and dehydration requiring IV hydration. RNs are required to titrate drips. Common titratable or set rate drips include: - Heparin - Insulin - Amiodarone - Cardizem - PCA fentanyl - Propofol - Norepinephrine Telemetry is monitored by techs, but RNs should know how to read rhythm strips in case of emergencies. Charge nurses take an assignment; house supervisors do not as they round throughout the building. There are nurse aides in the unit with a ratio of 1 aide to 8-10 patients during days and nights. Respiratory therapists and various equipment such as CPAP, BIPAP, Vapotherm, Servo U ventilators, and Hamilton ventilators are available 24/7. Shifts scheduled are 12 hours, with days from 0700 - 1930 and nights from 1900 - 0730. Weekend requirements include working every other weekend, and holiday requirements necessitate working every other holiday. Call or standby requirements may arise in case of low census, but cancellations are rare. Floating requirements include in-house to emergency and urgent care as needed. Scrub color for RNs is navy, and for aides, it is ceil blue. All clinicians must clock in and out at the facility.
