Saint Mary's Rd., Tucson, Arizona, United States of America Job Description Posted Wednesday, February 4, 2026 at 9:00 AM Marana Health is seeking a Staff Nurse Practitioner - Family Practice (FNP) promotes and maintains patients' health by providing medical services to all age groups to the extent which licensed and/or certified. The FNP uses independent judgment in carrying out daily job responsibilities. The position will be supervised by the Chief Clinical Officer . This FNP - Street Medicine role is uniquely focused on providing compassionate, community-based care to individuals experiencing homelessness or housing insecurity. The FNP - Street Medicine will be an integral part of our street medicine and outreach efforts, addressing complex medical needs that are often influenced by psycho-social challenges, social determinants of health (SDOH), and substance use disorders. The ideal candidate is passionate about serving vulnerable populations, comfortable providing care in non-traditional settings (such as encampments, shelters, and mobile units), and experienced-or willing to become waivered to prescribe medications for opioid use disorder (MOUD) . With the support of our integrated in-house behavioral health team, the FNP will work collaboratively to ensure patients have access to counseling and therapy services. This role is more than clinical-it's an opportunity to advocate, connect, and restore dignity through healthcare. It includes a twice-a-month weekend (Saturday or Sunday) field commitment, which is accounted for (offset) within the regular work schedule. REQUIRED QUALIFICATIONS: Family Nurse Practitioner master's degree or Doctor of Nursing Practice (DNP), with active license to practice in Arizona required. PREFERRED QUALIFICATIONS: Experience in a Federally Qualified Health Center (FQHC) or Primary Health Care environment preferred. Prior experience working with homeless, unstably housed, or substance-using populations is highly desirable. Equivalent combination of education and experience may be considered if applicable and must be directly related to the functions and body of knowledge required to successfully perform the job. KNOWLEDGE, SKILLS AND ABILITIES: Demonstrates a strong commitment to health equity, trauma-informed care, and patient centered principles. Experienced working with culturally diverse, low-income, ethnic minority, and marginalized populations, showing sensitivity and understanding of their unique needs. Possesses a deep understanding of social determinants of health (SDOH), community health frameworks, and public health issues in vulnerable settings. Familiar with harm reduction practices, wound care, and infectious disease management in underserved environments . Capable of quickly assimilating, assessing, and acting upon new concepts, trends, and initiatives in dynamic environments. Demonstrates excellent communication, interpersonal, and customer service skills, with the ability to build rapport under challenging conditions and represent the organization professionally across varied settings. Able to respond to questions from leadership, providers, managers, and healthcare team members with clarity and diplomacy. Skilled in reading, comprehending, analyzing, and applying complex documents, policies, and regulations in alignment with state and federal standards. Proficient in preparing, submitting, and documenting clear and concise reports using EHR systems and mobile documentation tools . Functions effectively and independently in field settings with limited resources, exhibiting flexibility, creativity, and resilience. Strong team player with effective collaboration skills and comfort working within interdisciplinary healthcare teams. DUTIES AND RESPONSIBILITIES: Follows evidence-based guidelines to diagnose, treat, and monitor acute and chronic conditions. Deliver comprehensive medical care in the field and clinical settings under the supervision of the Chief Clinical Officer. Provide primary care services including acute, chronic, and preventive care within the patient's environment (streets, encampments, shelters, mobile units). Build trusting, therapeutic relationships with patients who may be experiencing trauma, stigma, or chronic instability. Collaborate with social services, behavioral health, and outreach workers to address housing, food insecurity, addiction, and mental health needs. Utilize evidence-based guidelines to diagnose, treat, and manage medical conditions. Prescribe medications, including controlled substances and MOUD (when waivered). Order and interpret labs, imaging, and other diagnostic tests. Document encounters thoroughly in the EHR using mobile/remote access as needed. Refer patients to appropriate specialists and ensure care coordination. Participate in community events, outreach, and public health initiatives to enhance visibility and access. Identifies short-term and long-range patient care issues to be addressed, providing pertinent information, and recommends options and course of action. Promote patient education, self-care, and harm reduction practices. Participate in twice-monthly weekend outreach rotations, offset in the overall schedule. Collaborates with other providers in the care of patients as needed. Attend required CME training and maintain active licensure. Provides continuity of care by developing and implementing treatment plans. Diagnoses and treats common acute health issues. Diagnoses, treats, and monitors chronic health issues. Collaborates with physician as needed on complex, unusual, and high-risk cases. Orders and interprets diagnostic tests and prescribes proper medications. Follows up with patients through telephone, electronic, and mail correspondence. Documents patient contracts in patient chart . Performs outpatient procedures such as suture removal and other procedures within scope of practice. Promotes community awareness of health care issues. Promotes and participates in off-site activities such as health fairs, community, and school based programs as requested by Chief Clinical Officer. Ensures proper documentation in patient charting, patient encounter forms, insurance forms and all other additional information pertaining to patient care . Participates in other assigned duties as designated by the Chief Clinical Officer and/or Chief Executive Officer. Other duties/projects as assigned.
