Key Responsibilities

• Provide direct physical therapy services to patients in an outpatient clinical setting
• Evaluate, assess, and develop individualized treatment plans for patients
• Deliver therapeutic interventions in accordance with established clinical protocols and standards of care
• Complete required clinical documentation accurately and in a timely manner
• Collaborate with interdisciplinary care teams to support optimal patient outcomes

Required Qualifications

• Active Physical Therapist license — permanent Indiana state license required; compact license not accepted
• Prior experience as a Physical Therapist in a relevant clinical setting
• Ability to meet facility-specific credentialing and contract QA requirements prior to start
• Willingness to complete any facility-required documentation outside of standard onboarding clearance processes

Preferred Experience / Skills

• Experience in an outpatient physical therapy environment
• Familiarity with outpatient documentation systems and clinical workflows
