The successful Provider will perform essential duties in a variety of settings, including homes, facilities, telehealth, and clinics. Areas of Responsibility Evaluate high‑risk patients with complex medical needs and develop individualized care plans. Coordinate care activities with interdisciplinary team members to ensure continuity of care. Communicate regularly with case managers, coordinators, social workers, specialists, pharmacists, and health coaches. Participate in team meetings to discuss patient progress and care coordination efforts. Facilitate effective communication and collaboration to support seamless patient management. Maintain staff efficiency while delivering high‑quality care across various settings. Accurately document care management activities to support patient plans and regulatory compliance. Provide patient education and resources related to conditions, treatment plans, self‑management, and preventive care. Offer support and guidance to patients and families to enhance understanding of healthcare options. Adhere to all relevant healthcare regulations, policies, and procedures, including HIPAA and Medicare guidelines. Monitor quality measures, identify gaps, and implement corrective actions as needed. Qualifications Valid Nurse Practitioner (NP), Physician Assistant (PA), or Physician license in good standing in the state of employment. Demonstrated experience in patient care, preferably in home health, clinic, facility, telehealth, or specialty setting. Strong communication, collaboration, and interpersonal skills within interdisciplinary teams. Ability to evaluate high‑risk patients, develop care plans, and coordinate care activities to meet patient needs. Commitment to delivering compassionate, patient‑centered care and a culture of excellence. Strong organizational and time‑management skills, with the ability to multitask and prioritize effectively. Basic computer literacy. Leadership skills and ability to foster a cooperative work environment.
