Job Description

Perform physical therapy evaluation and treatment of patients based upon physician orders/referrals.
Complete comprehensive evaluations using appropriate assessment tools and procedures.
Establish measurable goals and develop treatment plans to enhance patient independence and function.
Regularly reassess treatment plans and modify as needed based on patient progress.
Communicate patient needs and progress to treatment teams and family members.
Maintain and submit evaluation, treatment, and billing documentation according to required standards.
Supervise Physical Therapist Assistants and Aides in accordance with state practice acts.

Critical Information

Shift: Monday - Friday 8am - 5pm EST, Onsite
County Covered: Boyd, Greenup, Lawrence, Carter, Elliot

Education/Licenses Needed

KY Physical Therapist Licensure, Compact Licensure Accepted
1 year home health physical therapy experience, previous oasis experience

Benefits:
