Responsibilities

• Establish and maintain relationships with acute care hospital staff, physicians, and other healthcare professionals to identify opportunities for collaboration and referral

• Conduct thorough assessments of patients' medical and social needs, identifying potential barriers to optimal care and developing personalized plans to address these needs

• Facilitate smooth transitions between care settings, ensuring that patients receive continuity of care and minimizing the risk of readmission

• Collaborate with interdisciplinary teams to develop and implement individualized care plans that address patients' physical, emotional, and social needs

• Provide education and support to patients and their families, empowering them to take an active role in their care

• Participate in quality improvement initiatives, identifying areas for improvement and implementing changes to enhance patient outcomes

Requirements

• 2 years of experience

• Strong communication and interpersonal skills, with the ability to build rapport with healthcare professionals and patients

• Ability to work in a fast-paced environment, prioritizing multiple tasks and deadlines

• Strong problem-solving and critical thinking skills, with the ability to analyze complex situations and develop effective solutions

• Familiarity with Medicare and Medicaid regulations, as well as industry standards for post-acute care
