Job Details Position: Physical Therapist (29-1123) – Home Health/Hospice. Reports to Clinical Manager of Home Health. Non‑Exempt Monday‑Friday, 8am‑5pm. Wage range $37.85 to $55.60 hourly. Sign‑on bonus $3000. Job Summary The Physical Therapist is responsible for assessing, planning, and delivering rehabilitative and strengthening programs that improve mobility, relieve pain, increase strength, and correct disabling conditions caused by disease or injury. Essential Functions / Major Duties and Responsibilities Provide physical therapy services (95%). Instruct patients and caretakers on disability management and obtain informed consent. Perform tests and measurements to evaluate disability, prognosis, and treatment options. Develop and implement individualized therapy, rehabilitation, and adaptation plans. Continuously evaluate and modify plans as needed. Prescribe, direct, and adjust adaptive or supportive equipment. Administer manual exercises, massage, traction, and physical agents (e.g., moist packs, UV/infrared lamps, ultrasound). Test and record patient strength, motor function, sensory perception, functional capacity, respiratory or circulatory efficiency. Provide functional training for daily living and negotiate practical situations. Conduct safety evaluations of patient homes, move furniture and belongings to promote safety. Coordinate care with physicians, therapists, home health aides, and other providers. Refer patients to outside resources and/or care providers when necessary. Document patient condition and treatment accurately in chart or electronic system. Monitor activities of other personnel involved in treatment. Attend case conferences (in‑person, telephonic, or electronic). Conduct in‑service educational programs for staff. Discharge patients when goals are achieved and arrange appropriate follow‑up or referral to community resources. Non‑Essential Functions / Other Duties Perform duties assigned to support RiverStone’s mission and goals. Education and Experience Minimum Qualifications Graduated from an accredited school of Physical Therapy. Experience or training providing equivalent knowledge, skills, and abilities for the role. Preferred Qualifications Experience in acute care, rehabilitation, neurology, or geriatric physical therapy. Experience working with underserved populations. Required Certificates, Licenses, Registrations Licensed Physical Therapist in the State of Montana. CPR Certification. Valid Montana State Driver’s License in good standing. Knowledge, Skills, and Abilities In‑depth knowledge of physical therapy principles and practices. Ability to care for diverse populations across age groups. Understanding of HIPAA and patient confidentiality. Working knowledge of computer programs and medical coding. Strong organizational and interpersonal skills. Calmness in emergency or crisis situations. Recognize and respond appropriately to urgent/emergent situations per protocols. Efficient multitasking abilities. Maintain a calm and positive demeanor during difficult interactions. Perform job duties with integrity and innovation to ensure high quality. Travel and use personal vehicle; drive in all weather conditions. Risk of needlestick injury possible. Freedom to Act & Decision Making Reports to the Clinic Manager for general direction. Responsible for determining content and effectiveness of treatment plans. Authority to modify services, make final technical judgments, and influence department functions.
