Compassionate Hospice RN / Case Manager
Gentiva
Gentiva Hospice is seeking an experienced RN Case Manager to lead nursing care in patients' homes, guiding families through the hospice journey with compassion. You will coordinate care with an interdisciplinary team and document assessments and Plan of Care to meet patient goals.
You will provide education and bereavement support, ensuring timely communication with the care team and adherence to regulatory standards. A valid RN license and CPR are required.
