POSITION SUMMARY - LCSW Social Worker


POSITION DUTIES - Conducts psychosocial assessments, coordinates discharge planning, facilitates communication with patients, families, and the interdisciplinary team, provides crisis intervention and supportive counseling, and connects patients with appropriate post-discharge mental health, medical, and community resources.


MINIMUM REQUIRED QUALIFICATIONS -

• MA LCSW
• Masters degree in social work
• CANS certification (must obtain prior to start)
• 2+ years recent social work experience in an acute hospital setting

PREFERRED QUALIFICATIONS -

• Charge capability
• Experience with older adults and/or working in a hospital with a psychiatric unit

LENGTH OF ASSIGNMENT - 7 weeks


SHIFT / HOURS PER WEEK - M-F 8:30a-5p - no weekends or holidays


START DATE - 8/31/2026
