Position Summary: Coordinate patient care from admission through discharge, ensuring that treatments, services, and resources align with each patient's medical needs and recovery goals. Collaborate closely with physicians, nurses, and external providers to remove barriers to care, streamline transitions, and support safe, efficient discharge planning. Utilization management and revenue integrity.

Position Duties:

• Collaborate closely with physicians, nurses, and external providers
• Remove barriers to care, streamline transitions, and support safe, efficient discharge planning
• Utilization management and revenue integrity

Minimum Required Qualifications:

• NH/compact RN license
• BLS
• 2 years case management experience in an acute hospital setting
• Acute hospital experience during the past year with no more than a 90-day break
• Critical care experience

Length of Assignment: 13 weeks

Shift / Hours Per Week: 8a-4:30p
