Responsibilities
- Perform comprehensive OT evaluations for residents in acute care contexts within a long-term care setting.
- Develop and implement individualized treatment plans to improve ADLs, mobility, strength, balance, and safety.
- Reassess progress regularly and adjust interventions; document outcomes and progress notes in accordance with regulatory requirements.
- Collaborate with an interdisciplinary team (nursing, therapy, physicians, social work) to coordinate care and discharge planning.
- Educate residents and families on therapeutic activities, home exercise programs, safety, and fall-prevention strategies.

Requirements
- Active Occupational Therapy license in Connecticut (as specified in Job Notes)
- Education: OT degree from an accredited program (as specified in Job Notes)
- Certification requirements as specified in Job Notes
- Experience requirements as specified in Job Notes
- Ability to work effectively in a long-term care setting and manage residents with acute needs
