Hospice Admissions Registered Nurse

Addus Family Company seeks a Hospice RN Admissions nurse to complete in-home hospice admission assessments and start-of-care visits. You will evaluate eligibility, educate patients and families on hospice services, develop initial care plans, and coordinate with physicians and the interdisciplinary team. This field-based role supports elderly and chronically ill patients to remain safely at home, using strong clinical judgment, compassionate communication, and accurate EMR documentation within a mission-driven, quality-focused organization.
