Hospice Registered Nurse (RN) Full Time On Call

Weekend On Call Full Time- Hospice RN

Advantage Home Health and Hospice is currently seeking a dedicated and compassionate Hospice Registered Nurse to join our team. As a Hospice RN, you will play a crucial role in providing comprehensive hospice care to patients in the comfort of their homes. This position will service the State College, PA and Surrounding Areas. You will work closely with a multidisciplinary team to ensure the physical, emotional, and spiritual needs of our patients are met during this challenging time.

What You'll Do

• Draft care plans, reconcile patient medications, and ensure orders for care are accurate and complete.
• Prepare accurate, orderly, and timely documentation.
• Coordinate care with the patient and provide proper reports to the Case Manager.
• Provide health care instructions to the patient/family as appropriate per assessment and plan.

About You

• You should have an active RN license in the state in which you are practicing, or be a new grad with eligibility to take NCLEX.
• Strong communication and interpersonal skills.
• Compassionate and empathetic approach to patient care.
• Ability to work independently and as part of a collaborative team.
• Reliable transportation and a valid driver's license.
