Job Level: Entry Level (less than 2 years) Join SSTAR’s Fall River Inpatient Team as a Case Manager (32 hrs/week) Are you passionate about helping individuals build a path to lasting recovery? SSTAR’s Fall River Inpatient Program is seeking a dedicated and motivated Case Manager to join our team! In this rewarding role, you’ll play a vital part in supporting patients as they transition from inpatient care to continued treatment and community resources. You’ll work closely with a collaborative, compassionate team to ensure each individual has the tools and connections they need for success beyond our program. What You’ll Do Provide comprehensive case management services focused on aftercare planning Connect clients with ongoing treatment, housing, and community resources Collaborate with a multidisciplinary team to support patient progress Help foster a safe, supportive, and recovery-focused environment What We’re Looking For A motivated, compassionate professional committed to making a difference Strong communication and organizational skills A team player who thrives in a fast-paced, purpose-driven environment Why Join SSTAR? Experience and Skills: Education: High school diploma or GED required; Bachelor's degree in Social Work or a related field preferred. Strong verbal and written communication skills. Open to working with people from diverse backgrounds, cultures and lifestyles in a non-judgemental manner. Direct care positions in addiction programs require two years of continuous sobriety for individuals in recovery.
