Case Manager Registered Nurse- Discharge Planner

The Home Care Coordinator Discharge Planner specializes in coordinating home care services and resources for patients transitioning from hospital to home, acting as a discharge planning resource without providing hands-on clinical care.

Work Location: Anne Adrundel , Annapolis Maryland

Weekly Working Hours: Days, 24 hours per week

Qualifications:

• Bachelor of Science in Nursing (BSN) required 
• Licensure in clinical discipline (RN) 
• Active CPR 
• Two (2) years experience in clinical nursing required , previous home care or home care discharge planning experience preferred
• Knowledge of Medicare Conditions of Participation preferred
• Thorough knowledge of home care community resources, agency policy and procedures, federal and state regulations and third party regulations for reimbursement of home care services preferred.

Salary Range: Minimum $39.64/hour - Maximum $61.45/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
