Travel RN Home Health - Menomonee Falls, Wisconsin

Schedule/Hours:

• 10-hour day shifts, 8:00 AM - 6:00 PM
• Shift options: Thursday-Sunday, or Saturday-Tuesday
• Guaranteed 40 hours per week (except during certain holidays or orientation)

Qualifications:

• Active WI RN license
• BLS certification (American Heart Association)
• Valid driver's license
• 1-2 years of recent home health and hospice experience
• Demonstrated proficiency with OASIS
• Recent experience with Epic charting (within 12 months)
• Strong time management skills; resilient, flexible, and social
• Physically able to manage stairs, uneven terrain, and drive between visits
• Able to work in homes with potential pet or smoke exposure
• Covid-19 vaccination required (medical/religious exemptions considered)

Key Responsibilities:

• Provide skilled nursing assessments and interventions in a home setting for adults managing chronic illnesses including CHF, COPD, diabetes, cancer, CVA, wound care, and IV administration
• Conduct admissions, death visits, or symptom management for hospice patients
• Deliver timely, accurate OASIS documentation
• Navigate varied home environments and rapidly changing schedules
• Maintain professionalism in both routine and challenging situations
