Responsibilities Demonstrate a commitment to community health, evidence-based practice, academic medicine, and patient-centered care. Adhere to applicable regulations for physician services and medical management. Initiate referrals for consult and specialty services as needed. Provide direct clinical services, including primary care and acute illness management, adhering to established protocols and evidence-based standards. Facilitate emergency and planned discharges to acute hospitals or medical treatment facilities. Ensure timely and accurate completion of medical records, orders, and documentation. Requirements Board Certified/Board Eligible within 5 years post residency required; Geriatrics preferred. Licensed in New York State. Passion for patient care and clinical excellence.
