Ohio Living seeks a Social Services Designee to plan, organize, implement, and evaluate social services for residents and patients within the life plan community. You’ll link underserved communities with resources and health care providers to meet emotional and psycho-social needs. You will collaborate with campus teams to ensure smooth care transitions, complete directives, and ongoing counseling as part of holistic patient care.
