Job Summary Provides direct and indirect patient care in the transitional care setting. Communicates with physicians, managers, and co‑workers about changes in the patient’s clinical condition, including diagnostic results and symptomatology. Responds quickly and accurately to changes in condition or reports on treatment response. Performs general nursing duties with supervision. Essential Job Duties / Responsibilities Knowledge of medications and correct administration based on patient age and condition. Performance of patient care responsibilities appropriate for each age group. Knowledge of principles of growth and development across the lifespan and skills to provide age‑appropriate care. Head‑to‑toe assessment and periodic reassessments per unit policy. Assessment and reassessment of pain; use of analgesia, including non‑pharmacologic techniques; patient and family education. Execution of all patient care in a safe environment to reduce medical errors. Formulation and revision of individualized plans of care; daily effectiveness evaluation. Implementation of teaching plans based on identified learning needs and evaluation. Delivery of treatments and services within scope of practice. Clear and appropriate communication with all healthcare team members. Consultation with other departments for interdisciplinary patient care. Provision of care tailored to disease condition and patient age. Assistance to physicians with procedures and manual skills. Flexibility, organization, and performance under stress. Respectful treatment of patients and families; addressing psychosocial, cultural, ethnic, and spiritual needs. Liaison between patients/families, physicians, other providers, and administration. Professional interaction with patients/families; patient/family involvement in care plan. Maintaining working relationships within and across departments. Documentation per current standards and policies. Communication of psychosocial needs to social services or discharge planners. Coordination and supervision of patient care as necessary. Adherence to HIPAA privacy regulations. Participation in interdisciplinary care meetings, departmental huddles, and care conferences. Transitional Care Specific Evaluation of patients at risk for falls and implementation of appropriate protocols. Use of nursing process for disease entities such as COPD, pneumonia, diabetes, anticoagulation therapy, total knee/hip replacement, cholecystectomy, CHF, UTI, MRSA infections, detox, cancer, and pain management. Participation in departmental performance‑improvement activities. Knowledge of standards for restraints and seclusion, including protocols, orders, documentation, and patient rights. Understanding of infection control, TB, blood‑borne pathogens, and hazardous waste plans. Position Qualifications Education / Experience / License and Certifications Current unrestricted Wyoming Registered Nurse licensure. Rehab nursing experience preferred. BLS certification required. Central Registry participation required. Additional Skills Effective communication in English (verbal and written). Additional languages desirable. Basic computer knowledge. Organizational skills.
