Visiting Nurse Association of Texas

In this role, you will be the first point of clinical contact, performing vital assessments and coordinating care to ensure patients and their families receive the support they need from day one. Job Summary

As an RN Admissions, your primary mission is to perform initial assessments and administer nursing care for clients in their place of residence. You will serve as a coordinator of care, collaborating with the interdisciplinary team, patients, families, and referring agencies to establish a seamless transition into hospice services.

Key Responsibilities

  • Patient Assessment: Evaluate hospice patients using Nursing Assessment and LCD criteria to identify physical, psychosocial, and environmental needs.
  • Care Planning: Develop and document comprehensive plans of care in correlation with initial assessments and infection control measures.
  • Clinical Coordination: Work closely with the Interdisciplinary Team (IDT), attending physicians, and hospice physicians to ensure all patient and family needs are met.
  • Documentation: Maintain accurate and timely clinical documentation, ensuring all paperwork is submitted according to state and federal regulations.
  • Direct Care & Productivity: Meet productivity standards by completing 2–3 admissions per day (or patient visits if admissions are unavailable) and participating in the on-call rotation as assigned.

Qualifications

  • Education: Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
  • Licensure: Current Texas State license as a Registered Nurse; current Texas Driver’s License.
  • Experience: Minimum of two (2) years of experience as an RN in a clinical care setting, and one (1) year of full-time hospice experience.
  • Skills: Strong interpersonal skills, expertise in generally accepted nursing standards, and knowledge of State and Federal hospice regulations.
  • Requirements: * Valid Texas Driver’s License, reliable transportation, and current auto liability insurance.
    • Ability to lift up to 50 lbs and move patients.
    • Proof of current CPR, TB testing, and Hepatitis acceptance/declination.
    • Ability to pass a criminal background check.

Working Conditions

This position involves traveling locally to visit patients in their homes and facilities. You should be comfortable working in various conditions, including occasional exposure to infectious diseases and unpleasant weather. The role requires prolonged standing, walking, and the ability to make quick decisions under stressful conditions to meet urgent patient needs.
