- Perform skilled nursing visits in patients’ homes
- Complete admissions, resumptions of care, recertifications, evaluations, and discharges as assigned
- Assess patients and develop or update individualized plans of care
- Administer medications and treatments according to physician orders
- Educate patients and caregivers about diagnoses, medications, and care plans
- Monitor changes in patients’ conditions and promptly report concerns
- Coordinate care with physicians, caregivers, therapists, and the interdisciplinary team
- Complete accurate and timely clinical documentation
- Follow agency policies and applicable state and federal regulations
- Active and unrestricted Texas Registered Nurse license
- Current CPR/BLS certification
- At least one year of nursing experience preferred
- Home health, hospice, or palliative care experience preferred
- OASIS and WellSky/Kinnser experience is a plus
- Valid driver’s license, reliable transportation, and current auto insurance
- Strong assessment, communication, and time-management skills
- Ability to travel throughout Fort Worth, Arlington, and surrounding areas
- Flexible PRN scheduling
- Supportive clinical and administrative team
