Pediatric Occupational Therapist – Birth to Five Center of Excellence Southwest Human Development (SWHD) is looking for Pediatric Occupational Therapists to join our Birth to Five Center of Excellence team. You will work with children birth to five, providing a combination of consultation and direct services. Services are provided in clinic, home and community settings within the Phoenix metropolitan area. Our Birth to Five Center of Excellence specializes in children birth to five with areas of expertise including complex developmental needs, feeding and eating problems, fragile infants, autism spectrum disorders, and developmental delays due to trauma. Responsibilities Provide services that are family‑centered and relationship‑based. Carry out screening and assessment activities with designated children in the areas of fine motor, self‑help, feeding, self‑regulation, coping and sensory integration skills. Choose, administer, score, and interpret standardized testing instruments appropriately. Integrate informal assessment, behavioral observation, developmental history, and other professional assessment data with standardized testing information to reflect a holistic view of the child and his/her needs. Participate in planning and implementing appropriate occupational therapy intervention. Design individualized goals and objectives that are developmentally appropriate, functional in nature, and related to ongoing routines of the home or class. Facilitate collaboration with parents, teachers, and other professional staff through regular contact, paperwork (e.g., progress notes), and designated team meetings. Facilitate a smooth transition for child and family into the next program placement. Demonstrate sensitivity to linguistic, cultural, social, economic, individual and role differences among persons and families served. Qualifications Licensed occupational therapist in the state of Arizona. Minimum of one year experience serving children under the age of five. Home visiting experience. Experience working within a transdisciplinary team (preferred). Experience providing consultation to community professionals (preferred). Possess an insured & reliable vehicle and a valid AZ driver’s license. Valid Arizona Fingerprint Clearance Card or qualify for a valid Arizona Level One Fingerprint Clearance Card. Education Masters or better in Occupational Therapy. Licenses & Certifications Auto Insurance Certificate. CPR and First Aid Certificate. Fingerprint Card Level 1. FPC DPS Verification. Occupational Therapist license. Driver License. DES Criminal Affidavit. DHS Criminal Affidavit.
