Nurse Practitioner

This position is designed for a Nurse Practitioner (NP) or Physician Assistant (PA) who wants to practice Outpatient Clinical Care in a supportive, collaborative environment. You will be fully integrated into the physician’s practice where you will: Work alongside a lead physician to co-manage a defined panel of geriatric patients Collaborate with a team of dedicated healthcare professionals, providing high-quality care to patients in an outpatient clinic setting. Perform preventative screenings and lead important discussions regarding long-term goals of care. Review and update patient medication lists to ensure accuracy, and order appropriate laboratory and diagnostic procedures Maintain and review patient records, charts, and other pertinent information accurately and efficiently. Maintain a high level of independent clinical decision-making in a fast-paced environment. Ensure patients receive care across the entire continuum and provide secondary support within various post-acute settings such as AL/IL and SNFs. Ideal Candidate Board-certified NP or PA Interest in geriatrics (experience preferred but not required) Comfortable in multiple care settings Strong communicator with patients and families Detail-oriented and thoughtful clinician Values mentorship and team-based practice New graduates with strong clinical rotations in geriatrics may be considered.
