Registered Nurse - Long-Term Care

Job Summary

Join a dedicated healthcare team at a non-profit health consortium serving the residents of Southeast Alaska, specifically in Wrangell. This Registered Nurse position is integral to providing comprehensive long-term care for residents of various ages, who may present with multisystem diseases and/or complications from treatment. The role focuses on delivering high-quality, patient-centered care within a supportive environment.

As a Long-Term Care RN, you will coordinate diverse nursing care procedures, make independent clinical decisions during your shift, and maintain accountability for your actions. You will play a key role in patient and family education, provide leadership by collaborating with ancillary nursing staff and other healthcare team members, and ensure professional nursing practice standards are upheld in the long-term care setting.

Key Responsibilities

• Provide safe, therapeutic, and efficient nursing care for residents, applying appropriate infection control measures and prioritizing care based on acuity and patient preference.
• Conduct comprehensive nursing assessments through interviewing, observation, and review of patient data, including age-related information, laboratory results, and diagnostic findings.
• Develop, implement, and evaluate individualized nursing care plans based on identified nursing diagnoses, scientific principles, and departmental policies, coordinating with physicians and the interdisciplinary healthcare team.
• Administer age-specific medications and therapeutics accurately, demonstrating proficiency in equipment use and monitoring.
• Educate residents, families, and staff on disease processes, procedures, and essential skills for coping with illness and promoting optimal health, evaluating the effectiveness of teaching.
• Act as a leader and resource, providing guidance and supervision to CNAs, LPNs, and Ward Clerks, and fostering a collaborative team environment.
• Communicate effectively and professionally with residents, families, staff, and other healthcare team members, providing constructive feedback and supporting new staff orientation.
• Document all aspects of resident care accurately and completely in the Electronic Health Record (EHR), including assessments, interventions, progress notes, and shift
