You'll support clients through clinical sessions, track outcomes against treatment goals, and connect people to higher levels of care when needed all from a remote setting.ResponsibilitiesConduct telehealth therapy sessions and ongoing clinical check-insEvaluate client needs, establish treatment goals, and monitor progress over timeDocument sessions, treatment plans, and updates in the EHRCoordinate referrals and community resources when clinically appropriateMaintain professional boundaries, confidentiality, and compliance standardsCommunicate clearly with internal teams as needed for continuity of care
