Patient Intake Coordinator ** On-site in Kalihi ** Overview

We are seeking detail-oriented and customer-focused Patient Intake Coordinators to support a fast-paced healthcare team. In this role, you will serve as the first point of contact for new patients, helping coordinate care and ensuring a smooth onboarding and scheduling experience.

This position is ideal for individuals who enjoy working in a high-volume call environment, managing multiple tasks, and delivering excellent service.

• Receive and review new patient referrals from hospitals and clinics
• Conduct outbound outreach to patients to coordinate dialysis care and scheduling
• Handle inbound calls from patients following up on scheduling requests (average 20/day)
• Manage patient cases through a Salesforce ticketing system
• Track and follow up on open cases, including patients who have not responded
• Document all communication and ensure cases are completed and closed accurately
• Maintain a high level of organization while managing a personal caseload

Outbound call volume will vary depending on workload (typically 20–80 calls per day).

• 1+ year of customer service experience (call center, retail, hospitality, restaurant, or medical office)
• Experience working in a healthcare setting (hospital, clinic, health insurer, or medical office)
• Strong verbal and written communication skills
• Excellent attention to detail and organizational skills
• Comfortable working with computer systems such as MS Outlook and data entry tools
• High school diploma or GED required

• Previous patient intake or care coordination experience
• Dialysis or specialty healthcare experience
• Familiarity with EMR/EHR systems (e.g., Cerner, Epic, Allscripts)
• Experience using Salesforce or similar CRM platforms

• Hybrid/in-office role with remote flexibility
• Company-provided laptop and monitor
• Must have:
  • Reliable high-speed internet (minimum 20 Mbps)
  • Private, distraction-free workspace
  • Ability to maintain patient confidentiality at all times

Strong team collaboration and communication are essential—this team values a positive attitude and great interpersonal skills.

• Full-time, Monday–Friday
• Standard hours: 8:00 AM – 5:00 PM or 9:00 AM – 6:00 PM
• Minimal overtime expected

• Contract-to-hire opportunity based on performance and business needs

This is a Contract to Hire position based out of Honolulu, HI.

The pay range for this position is $22.00 - $23.00/hr.

This is a fully onsite position in Honolulu,HI.
