Location:Parkway Surgery Center - ParkwayDepartment:Advanced AdmissionsWeekly Hours:0Status:Per DiemShift:Days (United States of America)Job Summary:ProMedica Toledo Hospital is seeking an Advanced Practice Provider in Advanced Admissions.Position Highlights:Shifts- Eight hoursSchedule- No holiday, weekend or on call rotationCurrent team consists of eight APP'sPatient Volume & Population- 16-21 patients/day (Pre-op/ H&P Service)Outpatient onlyACCOUNTABILITIES *All duties listed below are essential unless noted otherwise*- Performs patient assessment and evaluation as appropriate:Performs admission history and physical examination and records reports.Performs primary evaluations on patients.Performs developmental screening examinations as appropriate.- Manages the care and treatment of patients per scope of practice, level of competence, and current standards of care.Rounds on advanced admissions patients.Modifies management plan as appropriate.Analyses and interprets data, formulates problem lists, and develops comprehensive plans of care.Responds to calls from nursing and other direct care providers regarding changes in patient condition.Initiates consultation requests to physician specialists and to other health professionals as appropriate.- Develops and participates in quality improvement activities as outlined in SCA.Monitors effectiveness of care plans.Participates in chart review with collaborating physician.- Carries out appropriate documentation as required for safe clinical practice and to meet regulatory and organizational requirements.- Maintains professional licensure and certification. Participates in periodic clinical in-services.Attends annual conferences or completes self-study programs to achieve minimum professional requirements.Meets annual competency requirements.- Serves as liaison between medical, clinical and support staffs to improve quality of patient care delivery and increase operational efficiency and effectiveness.- Facilitates education process within department.Participates in formal and informal orientation of newly hired PAs and NPPs.Participates in education/staff development for nursing staff as request.- Performs other duties as requested.REQUIRED QUALIFICATIONSEducation: Master’s or Doctoral or a post-master’s certificate from a program that is recognized by a national nursing accrediting body that prepares graduates to practice as Nurse Practitioners or Physician Assistants. Must be eligible for Certificate of Authority to practice as CNP or PA in Ohio.Skills: Ability to work as member of multidisciplinary team is essential. Must be able to understand directions, communicate and respond to inquiries; requires effective interpersonal skills with a wide variety of individuals. Possess a high level of organizational, delegation and problem-solving skills. Must be able to input and retrieve information from computer. Must possess ability to design, implement, evaluate, and coordinate patient care programs and delivery approaches.Years of Experience: Previous experience in acute care setting.License: Must hold a current Ohio state license as a nurse practitioner or physician assistant. Must successfully complete hospital credentialing process.Certification: Must possess a current national certificate of registration to practice. Must be able to frequently move, lift or carry materials weighing up to 50 lbs., 20% of the time.
