What You'll Do

Deliver High-Quality Patient Care

* Provide direct physical therapy services under the direction of a licensed Physical Therapist * Follow individualized plans of care to help patients reduce pain, improve mobility, and restore function * Assist patients with therapeutic exercises, gait training, balance, and functional mobility * Support patients with transfers, strengthening, and activities of daily living * Utilize therapeutic modalities such as heat, cold, ultrasound, and electrical stimulation as appropriate * Educate patients and caregivers on exercises, safety, and use of assistive devices

Monitor Progress & Communicate

* Observe and report patient progress, response to treatment, and any changes in condition * Maintain accurate, timely documentation in the EMR * Communicate effectively with the Physical Therapist and interdisciplinary care team * Participate in case conferences and care coordination efforts

Collaborate & Contribute

* Work closely with nurses, therapists, and other healthcare professionals to ensure coordinated care * Participate in team meetings, in-services, and ongoing training * Uphold clinical standards and contribute to quality improvement efforts

What You Bring

* Graduate of an accredited Physical Therapy Assistant (PTA) program * Current PTA license or certification as required by state law * 1+ year of experience preferred (home health experience a plus) * Current CPR certification * Valid driver's license, reliable transportation, and auto insurance
