Job Title: RN Case Manager Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: 8H Days Hours per Shift: 37.5 hours per week Experience: Minimum 2 years License: NYS License required Certifications: BLS Certification required Must-Have: Flu vaccine required, no exceptions Description: The Medical Management Department is seeking an RN Case Manager to provide a range of direct and indirect services for patients and families. This includes identifying psychosocial problems and finding resolutions. The RN case manager collaborates with the patient, family, and team to ensure safe and timely discharge and treatment plans. Responsibilities include completing initial and concurrent insurance reviews. The RN will complete PRI for patients requiring skilled nursing facility placement. Psychosocial assessments will be made using all available data, including interviews with patients and families. Collaboration with interdisciplinary associates is essential to ensure continuity of care. The RN will develop, implement, and validate discharge and treatment plans in a timely manner. Independence is necessary, but supervision will be utilized to identify any issues in expediting discharge and treatment plans. Confidentiality must be maintained following hospital and social service policies. Ongoing linkages and networking relationships should be developed. Participation in patient-focused conferences and internal and external meetings is required. Leadership skills should be demonstrated by utilizing psychosocial expertise to assist with team building and discharge planning. The RN will educate interdisciplinary staff in conflict resolution and psychosocial matters when appropriate. Involvement in departmental programs such as quality improvement and continuing education is necessary. Knowledge of legal and ethical issues related to end-of-life care is required. This includes informed consent, the right to refuse treatment, and advance directives. Skills related to identifying, reporting, and intervening for victims of violence, abuse, and neglect are essential. Timely and safe discharge and treatment plans will be developed through sound psychosocial assessments. Workload organization should prioritize medical plans, patient needs, and departmental policies. Seeking help and resources to prevent unnecessary delays in service is crucial. Proactive advocacy and negotiation skills with patients, families, and community members are necessary. Demonstrating knowledge of the impact of medical, social, and environmental factors on patients and families is expected.
