Home Health PTA Physical Therapist Assistant Part Time ApplyRefer a FriendBack Job Details Requisition #: 215880 Location: West Palm Beach, FL 33409 Category: PT Salary: $33.75 - $41.25 per hour Position Details Paying $45-$55 Per Point Position Overview As an AveannaPhysical Therapy Assistantyou will utilize your clinical expertise to provide skilled education, treatment, and interventions as directed by the Physician in the home health plan of care. You will collaborate with interdisciplinary team members, receiving and providing pertinent updates to optimize patient well-being. PRN/Part-Time visits needed Monday - Friday for West Palm Beach, FL communities Essential Job Functions: u2022 Provides direct physical therapy according to directions of physical therapist and in accordance with the physicianu2019s plan of care. u2022 Communicates with and regularly updates the patient and/or caregiver regarding the plan of care. u2022 Provides skilled therapy services, treatments and procedures as appropriate for the patientu2019s care and safety. u2022 Reports observations of the patientu2019s condition to the designated supervisor(s), and/or the patientu2019s physician, as appropriate. u2022 Treats clients to relieve pain and/or develop or restore function to maintain maximum performance. u2022 Provides skilled teaching to the patients, family members, and caregivers regarding the safe use of prosthetic, orthotic and assistive devices (i.e., canes, walkers, wheelchairs, etc.) u2022 Educates patients and caregivers in carry over regarding issues concerning rehabilitation and adaptation techniques needed for recovery. u2022 Collaborates with other service disciplines to discuss the patient plan of care. u2022 Plans for clientu2019s discharge, prepares discharge instructions, and communicates discharge plan with patients, family members, and caregivers. u2022 Participates in in-service programs. u2022 Reacts to change productively and performs other job-related tasks and duties as assigned. u2022 Complies with ethical and professional standards of conduct as set forth by the American Physical Therapy Association u2022 Ensures confidentiality of patient records/information. Observes, records, and reports (1) signs and symptoms; (2) reactions to treatments and medications; and (3) any change in the patientu2019s physical or emotional condition to the patientu2019s physician and to the RN on the case if applicable, on a timely basis. u2022 Records clinical and progress notes at the time of the visit to ensure continuity of qualitative patient care on a timely basis. u2022 Completes daily report and any required documentation in a timely fashion to ensure agency operating schedules. u2022 Maintains consistent attendance. u2022 Submits all credentials in a timely manner. u2022 Performs all responsibilities in a professional manner Requirements: u2022 Gradation from a Physical Therapist Assistant education program from an accredited university/program. u2022 Current, unrestricted state license as a Physical Therapy Assistant in the state of practice u2022 Valid CPR As an employer accepting Medicare and Medicaid funds, employees must comply with all health-related requirements in all relevant jurisdictions, including required vaccinations and testing, subject to exemptions for medical or religious reasons as appropriate.
