Responsibilities

• Assess home health and hospice patients in stable and emergent situations to identify physical, psychosocial, and environmental needs.
• Implement, document, and evaluate the Plan of Care to ensure quality and continuity of care.
• Collect data on physical, psychological, social, spiritual, and cultural factors and incorporate that data into the Plan of Care.
• Maintain up-to-date patient records with visits, contacts, problems, plans, actions and goals; reflect changes promptly.
• Educate patients and families regarding care, medications, home safety, and self-care; adapt teaching to age, condition, culture, and readiness.
• Coordinate care with the interdisciplinary team and supervise/provide clinical directions to HHA/STNA/LVN as appropriate; arrange referrals when needed.

Requirements

• Current, active Registered Nurse license in the attending state (NC) - Required
• CHPN certification - Preferred
• CPR certification - Required
• Current driver’s license and reliable transportation - Required
• 1–2 years RN experience in a clinical care setting - Required
• 2+ years Home Health experience - Required
• Education: Associate Degree in Nursing or Nursing Diploma program; BSN preferred
