Make a meaningful difference while practicing with autonomy, flexibility, and a strong support team.The Nurse Practitioner provides home-based primary and palliative care to elderly, frail, chronically ill, and homebound patients in independent living, assisted living, and residential home settings within an assigned geographic territory.What We OfferCompetitive base salary with bonus opportunityFour-day workweekAssigned geographic territoryAutonomous practice with strong clinical and operational supportMeaningful, relationship-based care - not a traditional high-volume clinic settingMileage reimbursementPaid time off and paid holidaysMedical, dental, and vision benefitsMatching 401(k)Continuing education supportEstablished patient base and interdisciplinary care teamWhat You’ll DoPerform comprehensive medical evaluations and provide ongoing primary and palliative care.Manage chronic conditions, symptoms, medications, pain, and quality-of-life concerns.Develop and regularly review individualized care plans with patients, families, and caregivers.Order and interpret diagnostic testing as clinically appropriate.Prescribe medications and treatments within scope of practice.Educate patients and families regarding treatment options, advance care planning, hospice, and supportive services.Collaborate with nurses, social workers, hospice teams, caregivers, specialists, and other members of the care team.Coordinate safe transitions between home, hospital, skilled nursing, hospice, and other care settings.Maintain accurate and timely EMR documentation, including completion within 72 hours of visits.Provide coverage for other Practitioners within the care group as needed.Maintain compliance with organizational policies, professional standards, and regulatory requirements.Participate in continuing education and professional development.The Right FitThis role is ideal for a Nurse Practitioner who values independence, patient relationships, complex clinical care, and the opportunity to care for patients in the environment where they are most comfortable.Qualifications:Reliable TransportationMasters/Doctorate in Nurse PractitionerState License as Nurse PractitionerDEA LicenseCPR #J-18808-Ljbffr
