Zócalo Health is seeking an RN Case Manager to join our rapidly growing, remote-capable team in California. You will assess member needs, develop care plans, coordinate services, and support transitions of care within an interdisciplinary team led by a Nurse Practitioner. Responsibilities include conducting assessments, managing high-risk panels, and collaborating with Community Health Workers and Behavioral Health staff to deliver coordinated, whole-person care. #J-18808-Ljbffr Zócalo Health
